There are 2 versions of the 40 questions for self-diagnosis. The form below provides both sets of questions. The form will calculate a score (out of 40). There is no specific range that indicates that sex and love addiction is problematic for you – although higher scores are a stronger indication. The form is completely anonymous – when you complete the form you will be given the opportunity to download your answers as a PDF (or to edit your responses).

There is also a set of questions to help you ponder to what extent Sexual Anorexia may be an issue. The questions were revised and approved by the Annual Business Meeting in 2025, and these are the questions that are used in the below form.
The survey is anonymous. Nothing that may identify you, such as IP Address or location, are stored. Responses are auto-deleted 90 days after completion. You can edit or download your answers – but only until you close the browser window.
